Who It's For
Landlords who want a consistent way for tenants to report problems, and a simple repair history for each property.
How to Use It
- Give tenants a few copies of the request form at move-in (or email them the PDF).
- Ask tenants to use the form for non-emergency repairs. The form reminds them to call 911 or the utility company first for emergencies like fire, gas smells or flooding.
- When a request comes in, fill in the "Landlord Use Only" box: date received, who it's assigned to, scheduled and completed dates, and cost.
- Add each repair to the Repair Log on page 2.
- Keep completed forms and the log together. They're useful for planning and at tax time.
What's Included (2 pages)
- Page 1: Maintenance Request Form with tenant details, problem-type checkboxes, description lines, urgency, permission to enter, access notes, tenant signature, and a Landlord Use Only box
- Page 2: Repair Log (date reported, unit, problem, vendor, date fixed, cost, notes)
